Contract 2024

The contract form will be accessible on June 6

Your 2024 Memphis Suzuki Institute contract is in the below link.
1. Please review all the information and let me know if there are any errors and I will fix them and resend it.
2. For your security, I left the SSN blank and I will fill it in after you sign it.
3. Please print, sign (Contractor Signature), scan or take a picture with your phone, and return it to me in an email or text. Let me know if you have any questions, or if it is difficult for you to scan the document.

Course Sign-Off

Suzuki Violin Unit 1 form will be accessible on June 14

At the end of your course, please sign the top of the form, and initial each person who completed the course. If the trainee still has Requirements Not Met but did complete the course, you should still sign off on them. Once the roster is received at the SAA Office, the Staff will contact any Pending trainees to take care of their missing requirements so they can register for the course.
